Advance Housing staff provides flexible case management services to consumers transitioning from hospitalization or homelessness into permanent housing. The program helps consumers learn what is needed to successfully manage symptoms and obtain entitlements, education, and employment. The majority of services are provided to consumers in their own homes or community settings.
